Age milestone

Bereavement

Buying a home
  • OC only: Get your employment and income verification report from Equifax: The Work Number.
  • Speak to Empower for help in deciding to take a loan from your retirement plan account.
  • Review your retirement plan contribution election if you want to reduce or change the contribution type (Pre-Tax / Roth).
  • OC only: Check out PerkSpot for discounts to home products and services.
  • Update your will. Get assistance from MetLife Legal if you are enrolled in this benefit.
  • Update your tax withholdings on file with HR.
  • OC only: See what financial assistance is offered by local credit unions.
  • Schedule an appointment to speak to a financial advisor about your retirement account investments and other financial questions.
  • Update your contact information on file with HR.

Chronic condition
  • Vi​sit the Documents library to review the applicable leave of absence and sick leave policies for your region.
  • If you would like to start a leave of absence, contact:
  • Notify your manager if you will be taking time off.
  • If you are on a medical leave and enrolled in Short Term Disability, file a claim with Unum.
  • If you are not using PTO to cover your bi-weekly benefit plan premiums while on leave, make monthly premium payments to Tri-Ad.
  • If you are on a medical leave, file for State Disability insurance.
  • If you are enrolled in supplemental medical insurance, get your medical expenses reimbursed.
  • If you have a Health Care FSA or an HSA,submit your medical expenses to TRI-AD for reimbursement.
  • If you are enrolled in a Cigna medical plan, contact Cigna if you have any questions about your health benefits.
  • OC only: Under CHOC Fit, you have access to a range of services, including 1:1 personal lifestyle coaching, to support your nutrition, sleep, weight, musculoskeletal pain and mental health.
  • If you do not have PTO to cover your leave, contact the Benefits Department to learn about PTO donation availability for medical emergencies.
  • Access confidential counseling through the Employee Assistance Program (EAP).

Loss or gain of benefit coverage
(you or a dependent)
  • Enroll or change your benefits coverage, if necessary, within 31 days of gaining or losing other benefits coverage.
  • If are enrolled in the medical plan, you may be eligible to continue this coverage under ACA even though you lose your other benefits coverage. Contact the Benefits Department to find out.
  • If losing coverage of an RCH health plan, look to receive a COBRA packet from Tri-Ad.

Major repairs and natural disasters

Mental Health

Move
  • If moving out of state, obtain approval from your manager to continue your employment from the new state.
  • Update your contact information on file with HR.
  • Change your benefits, if necessary due to living in a different coverage area, within 31 days of your move.
  • Visit Cigna's website to find health care providers in your new location.
  • Update your state tax withholding information, if necessary.
  • OC only: Check out PerkSpot for discounts to entertainment, travel, services and products.
  • SD only: Visit LifeMart through the EAP for discounts.

This may be a qualifying life event, if you are moving to or from a location where your current medical plan does not cover your new location. Pets

Pregnancy and Children

The birth/adoption of a child is a qualifying life event. When you experience a qualifying life event, such as adding a new child to your family, you have 31 days from the date of your event to make benefits changes. See How to Enroll for instructions.

Retirement
  • Notify your manager.
  • Contact the Benefits Department to discuss your retirement benefit options.
  • Learn about your options for receiving Social Security benefits, if eligible.
  • Apply for Medicare, if eligible.
  • Speak to Empower for help in deciding when you may access your retirement plan savings.
  • Convert supplemental life insurance to individual policy, if desired, once you receive a conversion notification from Unum.
  • Access free, confidential counseling through the Employee Assistance Program (EAP).

After you’ve retired, you may be eligible for Medicare and/or Social Security benefits. Learn more on the Social Security website. You may also have the option of enrolling in COBRA coverage or your spouse’s benefits. Ask your spouse’s employer for their enrollment process and deadline.

Termination of Employment
  • Review the company's Benefit's Termination Letter.
  • If losing coverage in a health plan, look to receive a COBRA packet from Tri-Ad within 2 weeks after termination.
  • Schedule an appointment to speak to a financial advisor about your retirement account investments and other financial questions.
  • Supplemental life insurance ends on the termination date. Convert supplemental life insurance to individual policy, if desired, once you receive a conversion notification from Unum.
  • All accrued, unused PTO will be paid in your last paycheck in accordance with the applicable company policy.
  • Continue MetLife Legal plan through the portability option, if desired.
  • Convert Pet Insurance to an individual policy by completing a Continuation of Coverage form sent to you from MetLife after termination.
  • You have 90 days to submit any claims for reimbursement from your Health Care or Limited Purpose Flexible Spending Account (FSA) through Tri-Ad that were incurred up to your last day of employment.
  • Any funds in a Health Savings Account will remain yours.
  • OC only: Separation of employment or a termination event with a future date entered into Workday before a tuition reimbursement is processed will be cancelled in accordance with the Education Reimbursement Policy.
  • Short-Term Disability and Long-Term disability coverage with Unum ends on termination date.
  • OC only: Benefit-eligible associates not enrolled in a medical plan who are participating in CHOC Fit should reach out to Personify Health to redeem any unused Rewards Cash points within 30 days of termination.
Vacation
